Documents to accompany completed application
You may email these documents to HS-NPresidency@ucdavis.edu before or after you submit the application. They must appear in our email box no later than 11:59 a.m. Pacific Time (PST). Jan. 6, 2020. We recognize the holidays make it difficult to obtain letters of recommendation by the Jan. 6 deadline. The deadline for the letters of recommendation is now 8 a.m. Jan. 31.
- Resume or C.V.: must include all education, professional certifications and at least five years of work history. Applicants must provide a written explanation for any gaps in work exceeding three months.
- Three (3) letters of reference from people who can speak to your qualifications and capabilities as a nurse practitioner; one must be a supervisor. We recognize the holidays make it difficult to obtain letters of recommendation by the Jan. 6 deadline. The deadline for the letters of recommendation is now 8 a.m. Jan. 31.
- Personal statement of purpose (see information below). Please highlight your most relevant academic preparation and motivation, interests, specializations and career goals as well as background and experiences (see experiences and attributes above) for pursuing this primary care residency with a focus on underserved communities (1,000-word limit).
- Preparation and motivation may include your academic, clinical and/or research experiences that prepared you for this residency program. For example, you might speak to coursework, employment, exhibitions, fieldwork, foreign language proficiency, independent study, internships, laboratory activities, presentations, publications, studio projects, teaching and travel or study abroad along with your motivation or passion for advanced practice in primary care, particularly for the urban or rural underserved populations.
- Interests, specializations and career goals may include your research interests, disciplinary subfields, areas of specialization and professional objectives.
- Background and experiences may include any educational, familial, cultural, economic or social experiences, challenges, community service, outreach activities, residency and citizenship, first-generation college status or opportunities relevant to your academic journey; how your life experiences contribute to the social, intellectual or cultural diversity within a campus community and your chosen field; or how you might serve under-represented and underserved segments of society with your clinical education.
Applicants are encouraged to explore the following documents. These are requirements to start the residency but cannot be submitted prior to acceptance into the program. For those admitted into the program, specific information and assistance related to these documents will be provided during the preresidency onboarding day March 3, 2020. It will be helpful for you to be familiar with the documents.
- DEA1 certificate (can take 4-6 weeks to receive; must have N.P. License and Furnishing to apply)

- Proof of insurance provider status for Medicare3 and Medi-Cal4
- Satisfactory background check

The residency position is a limited-benefits, limited-term appointment with a duration of 12 months. NP-PRACTICE Residents receive a stipend*, two-weeks paid vacation and paid holidays.
Residents should expect to work primarily day and early evening hours, Monday through Friday; however, there may also be a required skills day or conference that takes place on a weekend day.
Residents are paired with an experienced physician or N.P. to provide clinical and practice guidance and support at the practice site. However, this is not a student experience. N.P. residents are expected to manage clinical patients with increasing independence through the year. Residents are also paired with an experienced N.P. professional role mentor.
Successful residents complete all 12 months of the residency, build a portfolio suitable to use to seek employment, receive a letter of recommendation for employers and are preferentially considered for open positions at UC Davis Health or potentially at the FQHC partner sites. N.P. residents also receive personalized employment assistance upon completion of the residency.
Residents cannot pursue employment opportunities until the last three months of the program and must complete the full 12-month residency.
